A Personal Support Worker (PSW) who is currently registered with HSCPOA.
Inactive
A PSW whose registration is currently inactive due to any of the following reasons:
Cancelled
A former HSCPOA PSW registrant whose registration was cancelled because:
- The Chief Executive Officer has been notified that the registrant is deceased, or
- The registrant has notified the Chief Executive Officer (CEO) that they are leaving Ontario, leaving the profession, changing professions, retiring, or provided another rationale not linked to compliance concerns and are voluntarily resigning as a registrant and will no longer be providing services.
Expired
The registrant's resignation has expired for failing to submit the annual renewal form by the required deadline.
Suspended
Suspension of a PSW registrant's registration by HSCPOA, with reasons noted.
Reasons:
- Suspension due to failure to pay fees,
- Interim suspension, based on reasonable and probable grounds that the conduct of the registrant poses risk of harm or injury to the public and/or those who receive health and supportive care services,
- Suspension directed by the Discipline Committee after a determination that a registrant has failed to comply with the prescribed Code of Ethics.
Revoked
A former HSCPOA PSW registrant whose registration was revoked, with reasons noted.
Reasons:
- Suspension for three years for failure to pay fees,
- Registrant meets the prescribed prohibited grounds for registration,
- Refusal to renew registration as registrant does not meet the prescribed criteria for that class of registration, or
- Revocation directed by the Discipline Committee after a determination that a registrant has failed to comply with the prescribed Code of Ethics.
Other Important Information
This may include:
- Relevant criminal charges,
- Findings of guilt,
- Court-imposed conditions and restrictions, and
-
Information about certain outcomes from CEO initiated investigations or complaints matters including:
- Written warnings,
- Orders to take additional educational courses or training, and/or
- A notation of any referrals to the Discipline Committee, including a copy of the specified allegations.
Discipline Committee Outcomes
All decisions of the Discipline Committee including any imposed disciplinary actions.
Previous Employment
Includes every location where the HSCPOA PSW registrant provided personal care services to the public prior to the Current Employment information but after the date the registrant was first registered with HSCPOA.
Where a registrant has resigned, retired, is deceased or has had their registration revoked, cancelled or otherwise terminated, their information in the HSCPOA Public Register shall be maintained for a period of 10 years. Information related to discipline proceedings in Ontario, will be maintained for a period of 50 years after the termination of registration.
